Kicking a Game Master while he's up
One of my gaming groups has to consider putting the axe to our Shadowrun Game Master. We operate on a swing schedule, him one week, me the other. He's missed two of his own games, both of which a majority of the group showed up for and found out he wasn't going to be there later on, and he missed the last session where he was a player. I basically haven't seen the guy in a month. The last session he ran he was distracted and under prepared (which generally does not work well with Shadowrun). The problem is that his life is finally getting back on track. He just got a good job and he (and his wife) just moved to a new place. I'm happy for the guy as another human being and that's why I feel a little bad we have to cut him loose. What it came down to, for us, was that if you can't run the game than you have to say so. We've asked him a few times if he can keep going and we received affirmations each time. Now, a month later, we had to make a choice as a group. So, yeah, a little fun. A guy's life improves and we kick him out of the group.

I can definitely see why you'd want to nix him as a gamemaster. We've had players like that who would disapear and not even bother to call. They usually had a good reason for not being there, but never a good reason not to reach in their pocket and use a their mobile phone.
I've never actually told a player to not bother coming back though. I relegate their character sheets to NPC's usually. Eventually, they come back in a story arc and die either nobly or horrifficly, depending on what the story needs. Players get a chance to get their act together before their character dies and if they come back after that (and they have) they make a new one.
If he gets less flaky, I'd consider letting him play again. If he tows the line for a good bit, he might even be able to run again. But that's my houserules for the 2 copper it's worth. :)
